Ruth Chepngetich Tested Positive for a Banned Substance: Marathon World Record-Holder Provisionally Suspended

The women’s marathon world record-holder, Ruth Chepngetich, is facing a furor of scrutiny and uncertainty after testing positive for a banned substance, sending shockwaves across the globe.

On Thursday, July 17, 2025, the Athletics Integrity Unit (AIU) announced that Chepngetich was provisionally suspended subsequent to her positive test for Hydrochlorothiazide (HCTZ), which is classified “as an anti-doping violation” because HCTZ is a prohibited diuretic according to the World Anti-Doping Agency (WADA).

Key Facts Regarding the Positive Test and Suspension

Chepngetich gave her positive sample on March 14, 2025 as part of an out of competition test. Apparently, the HCTZ level was close to 200 times over the reporting threshold. HCTZ is often utilized as a masking agent for other performance-enhancing drugs, but is typically prescribed for fluid retention and high blood pressure. HCTZ is prohibited in athletics unless there is a therapeutic use exemption (which Chepngetich did not have).

After being notified of the result in mid-April, Chepngetich, 30, cooperated fully with the investigators. The World Anti-Doping Code does not require a provisional suspension for diuretic cases, but Chepngetich voluntarily accepted a provisional suspension on April 19 while the AIU continued to investigate.

Chepngetich’s suspension comes just months after she made history at the Chicago Marathon in October 2024, finishing in 2:09:56—the first woman to ever complete a marathon in under 2:10. Chepngetich has also made her mark as a winner of the gold medal at the 2019 World Athletics Championships in Doha, particularly with her performance in such extreme conditions solidifying her standing as one of the greatest of the sport.

Her impact has inspired a generation of athletes in Kenya and beyond, which only makes the news of her provisional suspension even consequential across the fishing world.

Next Steps and Potential Outcomes

The AIU has also issued a formal Notice of Charge, so a disciplinary process is currently in progress. Chepngetich does have the right to be reviewed by an independent tribunal. If a violation of the anti-doping rule is established, she could receive a standard ban of two years, but it could be reduced if she can show some extenuating circumstance or inadvertent use.

The Chicago Marathon and World Athletics organizers stated again that they believe in strict anti-doping processes and will wait to comment until the AIU rounds out its disciplinary process.
